A GPs' group has called on politicians to "show a united front" over healthcare after auditors described a multi-million pound reform project as "more limited" than expected.
Transforming Your Care (TYC) was set up in 2011 with the aim of focusing more care in the community and reducing hospital stays.
However, the Northern Ireland Audit Office found that while some progress had been made in community mental health services and rehabilitation of stroke victims, the pace of change was "not as swift as originally envisaged".
Dr Grainne Doran, chair of the Royal College of General Practitioners Northern Ireland, said the report's findings were "deeply disappointing".
"General practice needs urgent support to make sure patients get the services they deserve," she said.
"We have been calling on the government to increase funding for general practice and ensure that it receives 11 per cent of healthcare funding. Today's report highlights that the shift in resource towards primary care has been much slower than necessary."
She added: "Our politicians need to work together and support health and social care reform.
"Political leaders must do the jobs they were elected to do and take action to save our health service."
