Scheme Coordinator, Choice Housing - Lisnavar Court

Are you a caring, sensitive and diplomatic person with recent experience providing care and support to people?Working together for positive change, Choice Housing is currently recruiting a scheme coordinator at Lisnavar Court. The post holder will provide day-to-day support to tenants - liaising directly with tenants, their relatives and Health and Social Services, and manage the scheme to ensure a safe and secure environment.You will sign up new tenants providing them with information, visit tenants regularly and liaise with families, medical staff, social workers, housing management staff, care staff and any other appropriate bodies. In this important and rewarding role you will also encourage the use of common amenities by tenants and neighbouring elderly people. You will also play a key role in ensuring the safety and wellbeing of tenants.

Northern Ireland Hospice is currently recruiting a hospice community nurse based in the Western Trust.Its vision is that infants, children and adults with life-limiting and life threatening illness and their families receive excellent and compassionate palliative care, thereby, maximising their quality of life. Working as a hospice community nurse, as part of a team of nurses, you will be responsible for the delivery of a specialist advisory service through the management of a caseload of patients within a community setting.The post holder will act as a resource to health care professionals within primary care, Northern Ireland Hospice multidisciplinary team, Trust multidisciplinary teams, agencies and individuals.This advisory and support role demands excellent clinical knowledge of specialist palliative care, effective communication skills and sound community awareness. The successful candidate will have the ability to work independently and as part of the community and extended hospice teams.
